How can I access Dignity House programs?

Accessing Care:

 

The Day Hospice Program

 

If you, a family member, a homecare worker, community nurse, or your physician are interested in learning how to access care at Dignity House Hospice, please contact us.

 

General Admission guidelines:

  • You have made the decision not to pursue further active treatment and are deemed palliative by your family physician;

  • You are comfortable with, and can follow our strict no-smoking policy;

  • You do not suffer from any type of dementia;

  • You are able to ambulate with minimal assistance;

  • You are not intubated and do not require suctiioning;

  • You are able to administer your own medications with little or no assistance;

  • You are able to sit in a comfortable chair or recliner for periods up to three hours with minimal discomfort.

Day Hospice services will be free of charge for all participants!
