Dignity House of Perth Hospice website!
Our
Mission
To
assist individuals diagnosed with
life-limiting illnesses to live life as fully as
possible while maintaining their dignity and
comfort, as well as to provide support to
their loved ones.
Our Purpose
The
purpose of Dignity House of Perth Hospice is
to develop and maintain a residential
hospice to provide for the palliative and
end-of-life care needs of patients diagnosed
with a life-limiting illness, and to support
their families and friends, in the Town of
Perth and surrounding communities --
specifically Lanark County.
By
choosing hospice care these patients have
made the decision, in consultation with
their family and doctor, to replace cure
goals with comfort and palliative goals.
As we work to gather the funding required to
undertake our final objective to establish the
residential hospice, the supporters of Dignity House plan to
offer other hospice programs to the
residents of Lanark County -- The Lanark
County Day Hospice Initiative, a
Bereavement Support Group, a Phone Support
Program, and eventually, an Outreach
Program.
All
services offered at Dignity House of Perth
Hospice will be patient-centered, and will
focus on the patient rather than the
illness, and will aim to give families
respite from the extraordinary challenges of
providing ongoing care to an ill or dying
loved one.
All support
services will be provided at no charge
to patients and their loved ones. Medical care will be provided
at no charge to patients holding a valid OHIP
card, and will be guided by a commitment to
EQUAL access to services for ALL residents
of Lanark County.
We take our patients’ confidentiality
seriously and ensure that it is protected at
all times.
Pain management will always be our number one priority.
We will work closely with our patients and their loved ones to
ensure timely referrals and advocate for appropriate intervention as
needed.
